This position is located in the Patient Registration Department at the Hopi Health Care Center (HHCC), in Polacca, AZ. This position assists the Benefit Coordinator and performs patient registration and eligibility services to new and returning patients. The incumbent also educates on alternate resources and interpretation of rules and regulations relative to the services.

Patient Services (85%)

  • Interviews patients and/or family member(s) in person or by telephone to obtain pertinent patient demographic information while maintaining patient confidentiality.
  • Assists patients in completing Patient Registration forms for Outpatient, Inpatient, and Emergency Room visits.
  • Assures and/or initiates timely data entry of all patient demographic, next of kin, emergency contact, tribe, employer, and insurance information into the RPMS Patient Registration System.
  • Interviews patients to obtain information to initiate a new medical record chart and works with Medical Records to retrieve a new record number, retired/stored record, maintain record keeping of logs, and enters information into the RPMS Patient Registration System.
  • Prints Master Patient Index cards (MPI), new patient identification addressograph cards, etc. and provides to Medical Records to ensure they are scanned and/or filed.
  • Determines eligibility of patients seeking health care services, who have not previously been treated at the facility to determine if they are eligible for “Direct Care” or “CHS and Direct Care”, by obtaining the Certificate of Indian Blood (CIB) or other documentary proof of tribal membership, according to IHS guidelines. Ensures the patient understands their fair entitlement in non-IHS/IHS facility.

Other Duties (15%)

  • Interviews family to complete the Death Registration Worksheet to collect pertinent information regarding the death of a family member. Completes the Death Registration Worksheet, Human Remains Release Form (HRRF) according to State/County regulations. Fax completed worksheets and HRRF to the County Vital Records Office according to regulations.
  • Enters decedent’s information into the D.A.V.E system to initiate the death certificate process with the County Public Health Services. Maintains copies and a log of all entries complete.
  • Interviews family to complete the Certificate of Live Birth Worksheet to collect pertinent information for a birth certificate, including obtaining signatures and social security numbers required by rule.
  • Completes the Live Birth Worksheet and submits to the local registrar, a deputy local registrar or state registrar.
  • Enters the child’s information into the EBRS system to initiate the birth certificate process with state to register the birth of the child.
  • Maintains copies and a log of all entries complete for ten years.
  • Incumbent maintains a daily log of workload.
